The United States Property & Fiscal Office- Kansas, Kansas Army National Guard (KSARNG) is seeking current and relevant experience, personnel with the capability to perform Health Care Services in support of the Kansas Army National Guard at Lenexa, Salina and/or Wichita Kansas. This is not a request for offers or proposals but is a survey of the market for potential sources. No reimbursement will be made for any costs associated with providing information in response to this announcement or any follow-up information requests. Not responding to this notice does notpreclude participation in any future RFQ, IFB, or RFP, if any is issued. If a solicitation is released, it will be synopsized on the Government Wide Point of Entry (GPE). It is the responsibility of potential offerors to monitor the GPE for additional information pertaining to this requirement. The GPE is located at https://beta.sam.gov. All interested sources should respond. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. We encourageall small business concerns in all socioeconomic categories (i.e. Small Disadvantaged Business(SDB), 8(a), Historically Underutilized Business Zones (HUBZone), Service-Disabled Veteran-Owned Small Business(SDVOSB), Economically Disadvantaged Woman-Owned Small Business (EDWOSB),and Woman-Owned Small Business (WOSB)) to identify their capabilities in meeting the requirement at a fair market price.

Contractor tasks include, but are not limited to the following: Annual dental exams, dental x-rays, real-time DENCLASS data. Periodic Health Assessments (PHAs), Soldier Readiness Programs (SRPs), Specialty Physical exams including Flight physicals, women’s health requirements, laboratory and phlebotomy services, urine and blood tests, immunizations, visual acuity and Optometry exams, MEDPROS entry. Services to be completed on site throughout Kansas. Optometry services to include visual refraction, measuring papillary distance, frame measurement, and other testing needed. Conduct and complete requirement of the KSARNG Medical and Dental Readiness on all eligible Service Members utilizing HIPAA, ADA, Privacy Act, and OSHA compliant Mobile Health/Dental practice provided by the Contractor. Medical services will be performed by licensed/registered/certified medical professionals with oversight by the KSARNG. Dental treatment will be conducted in an OSHA & ADA compliant mobile or fixed dental facility provided by the contractor. Medical evaluations will be conducted bya licensed provider (physician, physician assistant, or nurse practitioner) licensed to practice within the State of Kansas.

The NAICS code assigned to this acquisition is 621999, AllOther Miscellaneous Ambulatory Health Care Services, with a corresponding size standard of $16.5 Million. The estimated single award Indefinite Delivery/Indefinite Quantity (IDIQ) contract value is $6.0 Million with a One Year Base and four One Year Options. The estimated minimum guaranteed amount is $5,000.

Capability packages must be submitted by no later than Thursday, November 21, 2019 and should include the following:

1. 1. A company profile including company name, point of contact, phone number, email address, number of employees, CAGE code and DUNS number. Respondents must indicate their business size.

2. 2. Provide your company’sspecific experience in performing Dental and medical exams for Military organizations and providing comparable services. Provide details regarding previous experience in performing dental screening examinations and periodic health assessments, any other types of services provided, mobile/portable facilities available, and any other relevant information youdeem applicable.

Elaborate brochures or documentation are unnecessary and are not desired.
