Brief Service Description: Contractor shall provide troubleshooting, repair, perform preventive maintenance inspections (PMI), and replacement of defective parts. Services are for a St. Jude Medical EnSite Velocity Cardiac Mapping System,WorkMate Claris with EP-4 Cardiac Stimulator and a ViewMate Ultrasound Imaging Console Support Systems, serial numbers 15706245, 15718366 and 30474C4151. These systems are located in room C623 at South Texas Health Care Systems; Audie L. Murphy Memorial Veterans Hospital; 7400 Merton Minter Boulevard, San Antonio, Texas 78229-4404. Contractor is to provide 100 percent of all materials, tools, travel, and labor. Adherence to Joint Commission Accreditation Healthcare Organization (JCAHO), Original Equipment Manufacturer s standards and the Food and Drug Administration (FDR) Code of Federal Regulation (CFR) Title 21 is mandatory.

This action is accomplished under FAR 13.106-1(b)(1)(i), Soliciting from a Single-Source . This notice is not to be considered a request for quotations or proposals. No contract will be awarded based on offers received in response to this notice; however, any offers received may be used to determine if a comparable source is available and more advantageous to the Government. All responsible sources may submit a response; which, if timely received, will be considered by the agency. If parties choose to respond, any costs associated with the preparation and submission of data or costs incurred in response to this announcement shall be the sole responsibility of the contractor and will not be reimbursed by the Government. A determination by the Government not to compete this proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no affirmative written responses are received by the responsedate and time, a purchase order will be issued to Saint Jude Medical Inc. without any further notice.
